FOREIGN BODY IN NOSE

This is one of the common emergencies in children below five years. These children have habit of putting any article in the nose and they do not realize the consequences of such act.

The common articles which are put in nose are food particles- like grains, pencil, eraser, pearl, button, small stone or sand. It is a common experience that re- moval of such foreign body is tried first at home by parents. If they are unsuccessful or when it goes further inside the child is brought to the Gen- eral Practitioner. Rarely the child may be brought after 2 to 3 days with symptoms like blockage of nose and bleeding from nose.

 Examination of Child

Child’s head should be hold properly to minimize the movements. Powerful beam of light is required to see the nature and exact size of foreign objects. There may be inflammation of nasal mucosa, which may bleed on touch.

Treatment of Foreign body

Proper visualization of foreign body, skill and confidence is required for removal of foreign particles; otherwise one may push the foreign particles further inside. Rarely it may get pushed deep inside and blocking the air passage.

Nasal foreign paeticles remover should be used. Gently try to go beyond the foreign body and pull it downwards. For safety purpose try only once and if the foreign body doesn’t come out refer patient to ENT surgeon, but never be over confident about removal of nasal foreign body, because it may lead to serious complications.

FOREIGN BODY IN EYE

Child may be brought with the history of (i) watering of eye; (ii) Discomfort: Slightly older child may complain about the symptom but infant may present with excessive rubbing of the eyes.

Examination-

Allow the child to settle in clinic be- cause child’s co-operation is required for detection of foreign Body in the eye. Good torch light is required to detect the foreign body. Ask mother to retract upper eye lid and you re- tract the lower eye lid with one hand and use torch light with the other hand. One can get good co-operation from grown up children.

Treatment –

Small conjunctival foreign bodies, situated superficially can be removed with cotton tipped applicator, soaked in water. Child with corneal foreign body should be referred to Ophthalmologist for further treatment.

FOREIGN BODY IN EAR

Pencil, eraser, sand, grains, insects, these are the common foreign particles in ear. putting foreign particles in the ear; (ii) The child may present with (i) H/o Pain in ear; (iii) Bleeding from ear; (iv) Sometimes blocking of impacted .hard wax can also present like foreign particles in air.

Examination

Otoscope or powerful beam of light is required to visualize the foreign body. Sometimes the ear drum may not be visible because of foreign particles.

Treatment –

1. Larger foreign particles in ear can be removed with forceps. 2. Fine particles such as sand, grain can be removed by syringing of the ear. 3. Anesthetic drops should be put in the ear to prevent the severe pain in ear.
